Regression as Checks

The in-house procedure for handling repeat mistakes. The default response to a mistake happening twice was one more caution line in the reference document, and a few months of that is several hundred lines nobody reads to the end.

Built a procedure that moves repeat mistakes out of a cautions document and into machine-enforced checks. Capture, classify, promote, retire: retirement is part of the procedure so the ledger does not only grow. What the checks cannot cover is written down as uncovered.
